What To Expect On Your Initial Office Visit

In order to provide you with an optimal experience, we ask that all new patients arrive 20-30 minutes before your appointment time to complete a new patient information packet. Please sign-in with the receptionist upon arrival to the clinic. The receptionist will need a copy of your insurance card and will ask you to complete a new patient profile. Dr. Dalton-Smith’s nurse will call you back to the exam room at your appointment time. We realize that your time is valuable and we strive to make your wait time as short as possible. Please bring all current medication bottles to every visit for identification and verification of prescriptions. Anniston Medical Clinic is a comprehensive health care facility with most labs, x-rays and screening test being done on-site. The clinic has multiple internists as well as several specialists (rheumatology, pulmonology, and hematology/oncology) on staff to meet your health care needs.
